Simply shop at 5 different participating Maps-2-Memories Stores, spend $20.00 or more in each Store, and then mail us your receipts. Then, we'll send you a Maps-2-Memories key tag (as seen above)  good for a LIFETIME 5% discount* in ANY of our stores nationwide. What could be easier?

Once you have shopped at 5 different Maps-2-Memories stores and spent a minimum of $20.00 in each, print out this form and send it, along with your 5 receipts to:
